1) there is no reason to add yeast to wine, since "wine" is already fermented, no additional fermentation is needed. Btw, unfermented grape juice is called "must" - if you are asking how much to add to the must, I would say one whole vial.
2) Packet? If you are using yeast in a packet from the grocery store, the only thing you will make is grape juice that tastes like bread. If you are serious, go to a winemaking store and get a yeast especially made for fermenting wine (not making bread rise).

Add the whole packet. You can make 5 gallons with the packet but the extra yeast won't damage the wine and it won't keep once it's opened.

Add the packet or vial to the juice. Theoretically, you could save some but it doesn't really pay.
BTW, IVV, wine yeast comes in packets too.
